It's a Mandrake Style known as Galaxy.....
I found it from Mandrake Linux 10.0..
It's nice and clean...........
copy Galaxy.kcsrc ---------->/usr/share/apps/kdisplay/color-schemes/
copy galaxy.themerc--------->/usr/share/apps/kstyle/themes/
copy galaxy.la and galaxy.so ------>/usr/lib/kde3/plugins/styles/
now
for color:
open "kde control center" select "appearence & themes" select "color" select linuxcolor
for style:
open "kde control center" select "appearence & themes" select "style" select thememdk

what's the iconset ?
There are two iconset i use 1.crystelsvg replacement icon http://personal.telefonica.terra.es/web/ivanciko/crystalsvg.tar.bz2 2.and the toolbar icon wher i found i forget but if u wish i may upload these icons.
